If your child requires medication during school hours you must  follow the steps below:

For Prescription Medication:

1. Complete the Provider/Parent Authorization Form. This form must be signed by parent or guardian and healthcare provider who is prescribing the medication. This permission form is valid for one school year. 

2. Medication must be brought to the school nurse or trained medication assistant by an adult. Never send medication to school with your child! Scheduled drugs must be counted and documented by the parent/guardian and the nurse, or the trained medication assistant.

3. Medication must be in the original container with a  clear/complete pharmacy label.

4. Notify the nurse and complete a new form if the medication or the prescribed dosage changes.

5. If the prescription is stopped, please notify the nurse and pick up any remaining medication.

6. Only medications that are required to be given during school hours will be administered by the nurse or medication assistant, otherwise medications should preferably be given at home by the parent or guardian.

Non-Prescription Medications or Over-the Counter Medications:

1. Must have a Medication Permission form completed.

2. Medication must be brought to school by the parent/guardian or designated adult. Must be brought in the original container with the name of the student clearly written on the container.

3. Permission forms must be renewed each school year for prescription and non-prescription medications. Any remaining medication should be picked up by the parent/guardian at the end of the school year. Medication not picked up by parent will be disposed of according to accepted guidelines.
